Depresyon ve anksiyete bozukluğu olan ebeveynlerin çocuklarında görülen ruhsal bozukluklar

Abstract

Amaç: Majör depresif bozukluk (MDB) veya anksiyete bozukluğu (AB) tanılı ebeveynlerin çocuklarında yıkıcı duygudurum düzenleyememe bozukluğu (YDDB) başta olmak üzere ruhsal bozuklukların araştırılması amaçlanmıştır. Yöntem: Mersin Üniversitesi Tıp Fakültesi Psikiyatri polikliniğine Eylül 2018-Eylül 2019 tarihleri arasında başvuran MDB olan 46 ve AB olan 34 ebeveynin 6-18 yaşları arasındaki 80 çocuğu çalışmaya dâhil edilmiştir. Kontrol grubunu, herhangi bir psikiyatrik rahatsızlığı olmayan Mersin Üniversitesi Tıp Fakültesi pediatri polikliniğine basit fiziksel yakınmalar ile başvuran sağlıklı ebeveynlerin aynı yaştaki 40 sağlıklı çocuğu oluşturmaktadır. Çocuk ve ebeveyn ile Okul Çağı (6-18 Yaş) Çocukları İçin Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi-Şimdi ve Yaşam Boyu Versiyonu Türkçe (ÇGDŞ-ŞY-T), Beck depresyon, Beck anksiyete, DSM-IV’e dayalı DEHB tanı ve değerlendirme ölçeği, Çocukluk Çağı Kaygı Bozuklukları Ölçeği (ÇATÖ), Çocuklar için Depresyon Ölçeği (ÇDÖ) uygulanmıştır. Bulgular: Çalışmamızda ebeveyninde AB ve MDB olan çocuklarda daha fazla ruhsal bozukluk saptandı. En sık görülen psikopatoloji DEHB iken bunu YAB, MDB, sosyal fobinin takip ettiği görüldü. Tüm örneklemde DSM-5’e göre sadece bir olgu (%2.9) YDDB tanısını karşıladı. Hem MDB hem AB grubunda karşıt olma karşı gelme puanları (p=0.019), AB grubunda ise davranım bozukluğu puanları (p=0.043) kontrollerden anlamlı düzeyde yüksekti. Sonuç: Ebeveynlerinde ruhsal bozukluk olan çocuklar psikopatoloji gelişimi açısından risk altındadır ve ebeveynlere yönelik uygun tarama programlarının geliştirilmesi önem arz etmektedir. Çocuk psikiyatri kliniklerine en sık başvuru şikâyetleri sinirlilik, öfke patlamaları gibi YDDB semptomları iken çalışma sonuçları tanısal anlamda YDDB’nin düşük olduğunu göstermiştir. Bu durum ileri çalışmalarda daha çok YDDB’nin semptomlar düzeyinde irdelenmesinin faydalı olabileceğini düşündürmektedir.

Psychopathologies in children whose parents are diagnosed with a major depressive disorder or anxiety disorder

Abstract

Aim: This study aims to investigate psychiatric disorders, especially disruptive mood dysregulation disorder (DMDD), in children whose parents have a major depressive disorder (MDD) or anxiety disorder (AD). Method: The sample consisted of 80 children whose parents were admitted to the Psychiatry Outpatient Clinic in Mersin University Faculty of Medicine and diagnosed with an MDD (N=46) or AD (N=34) between September 2018 and September 2019. The control group consisted of 40 aged-matched healthy children whose parents did not have any psychiatric disorders. These children were admitted to the Mersin University Faculty of Medicine Pediatric Outpatient Clinic with simple physical complaints. Schedule for Affective Disorders and Schizophrenia for School-Age Children- Present and Lifetime version- Turkish, Beck Depression Inventory, Beck Anxiety Inventory, the Turgay DSM-IV Based Disruptive Behavior Disorder Rating Scale, The Screen for Child Anxiety Related Disorders, and Children’s Depression Inventory were used to evaluate psychiatric symptoms of children with their parents. Results: The children whose parents had an MDD or AD showed more psychiatric problems than healthy controls. ADHD was the most frequently observed diagnosis, followed by GAD, MDD and social phobia. According to DSM-5, only one case (2.9%) met the diagnosis of DMDD in the whole sample. The scores of oppositional defiant disorder in the MDD and AD group were significantly higher than in the control group (p = 0.019). Additionally, conduct disorder scores in the AD group were significantly higher than in the controls (p = 0.043). Conclusion: Children whose parents have a mental disorder, are at risk for developing psychological problems and it is important to develop appropriate screening tools for parents. While the most common complaints of admission to child psychiatry clinics are DMDD symptoms such as nervousness and temper outbursts, the study results showed that the diagnostic rate of DMDD was low. This situation suggests that it may be useful to examine DMDD at the level of symptoms in further studies.

MEU Journal of Health Sciences Assoc was began to the publishing process in 2008 under the supervision of Assoc. Prof. Gönül Aslan, Editor-in-Chief, and affiliated to Mersin University Institute of Health Sciences. In March 2015, Prof. Dr. Caferi Tayyar Şaşmaz undertook the Editor-in Chief position and since then he has been in charge.

Publishing in three issues per year (April - August - December), it is a multisectoral refereed scientific journal. In addition to research articles, scientific articles such as reviews, case reports and letters to the editor are published in the journal. Our journal, which has been published via e-mail since its inception, has been published both online and in print. Following the Participation Agreement signed with TÜBİTAK-ULAKBİM Dergi Park in April 2015, it has started to accept and evaluate online publications.

Mersin University Journal of Health Sciences have been indexed by Turkey Citation Index since November 16, 2011.

Mersin University Journal of Health Sciences have been indexed by ULAKBIM Medical Database from the first issue of 2016.

Mersin University Journal of Health Sciences have been indexed by DOAJ since October 02, 2019.

Article Publishing Charge Policy: Our journal has adopted an open access policy and there is no fee for article application, evaluation, and publication in our journal. All the articles published in our journal can be accessed from the Archive free of charge.
